L'mwbgiperalgesia è una condizione di aumentata mwbwsensibilità al mwcadolore, che si manifesta in caso di stimoli lievi a causa di una lesione dei mwcqnocicettori o dei mwcgnervi del mwcwsistema nervoso periferico. Un temporaneo aumento della sensibilità dolorosa può essere notato anche nel cosiddetto mwdasickness behavior, il comportamento adottato dal mwdgcorpo umano in risposta alle mwdwinfezionicite-ref-hart-1-0[1].

Il termine deriva dal greco ὑπέρ, oltre e da ἄλγος, dolorecite-ref-hart-1-1[1].

Classificazione

L'iperalgesia può manifestarsi in mwhaaree cutanee limitate in numero e dimensione oppure assumere una forma generalizzata. Studi sul condizionamento hanno stabilito che è possibile sviluppare una forma di iperalgesia diffusa "imparata" e non basata su meccanismi eziopatogenetici fisici. La forma focale, tipicamente associata a mwhqtraumi fisici, può essere suddivisa in una forma primaria, che si manifesta nell'area direttamente lesionata e in una forma secondaria, che si presenta invece nei tessuti circostanti non lesionati. Esiste anche una forma indotta da mwhgoppioidi, che può svilupparsi dopo una somministrazione prolungata nel trattamento del mwhwdolore cronico, confermata da studi condotti su esseri umani e animali e che può comportare la sospensione del farmacocite-ref-chu-2-0[2].

Eziologia

L'iperalgesia è causata dal mwjgfattore attivante le piastrine (PAF) rilasciato durante la risposta mwjwinfiammatoria o mwkaallergica dalle cellule del mwkqsistema immunitario che reagiscono con i mwkgneuroni del mwkwsistema nervoso periferico attraverso il rilascio di mwlacitochine e mwlqchemochine coinvolte nel meccanismo di produzione del dolorecite-ref-3[3].

La stimolazione dei mwmwrecettori dolorifici in corso di infiammazione può portare a un'amplificazione del segnale con un meccanismo di mwnapotenziamento a lungo termine che si sviluppa a livello del mwnqmidollo spinale coinvolgendo la mwngsostanza grigia periacqueduttale del mwnwmesencefalocite-ref-ikeda-4-0[4]. Il rilascio di citochine pro-infiammatorie, quali l'mwpainterleuchina 1, da parte di mwpqleucociti attivati dal contatto con il mwpglipopolisaccaride, con alcune mwpwendotossine e altri messaggeri di infezione risulta coinvolto nell'aumento di sensibilità dolorosa che si può riscontrare nel mwqasickness behaviorcite-ref-hart-1-2[1]cite-ref-5[5]cite-ref-6[6].

L'utilizzo cronico di oppioidi sia come mwtwsostanze stupefacenti, sia nel trattamento del dolore cronico, può condurre a iperalgesia e al riscontro di esperienze estremamente dolorose in proporzione al riscontri clinici, cosa che rappresenta la più comune causa di perdita di efficacia del mwuafarmacocite-ref-chu-2-1[2]cite-ref-7[7]cite-ref-8[8]. È difficilmente distinguibile dalla mwxqtolleranza, in quanto anche l'iperalgesia da oppioidi è compensata dal paziente mediante un aumento del dosaggio del farmaco, condizione che può potenzialmente peggiorare il problema portando a una sempre maggiore sensibilità al dolore. La sovrastimolazione cronica dei mwxwrecettori oppioidi porta a un'alterazione dell'mwyaomeostasi delle vie di trasmissione nervosa del dolore, soprattutto quella mediata dal recettore della nocicettina (o recettore NOP)cite-ref-9[9]cite-ref-10[10]cite-ref-11[11], tanto che si ritiene che bloccando questo recettore si possa prevenire lo sviluppo dell'iperalgesiacite-ref-12[12].

Una causa non comune di iperalgesia è il mwcwveleno di mwdaornitorincocite-ref-13[13].

Trattamento

L'iperalgesia è una condizione simile ad altre sindromi dolorose quali l'mwewallodinia e il mwfadolore neuropatico e risponde alla terapia di riferimento per queste entità nosologiche. Questa può prevedere la somministrazione di mwfqantidepressivi triciclici o mwfgSSRIcite-ref-14[14]cite-ref-15[15], mwhwFANScite-ref-16[16], mwjaglucocorticoidicite-ref-17[17], mwkqgabapentincite-ref-18[18] o mwlgpregabalincite-ref-19[19], mwmwantagonisti del mwnarecettore NMDAcite-ref-20[20]cite-ref-21[21]cite-ref-22[22] oppure oppioidi atipici come il mwqqtramadolocite-ref-23[23].

Nei casi di iperalgesia indotta da oppioidi, la riduzione del dosaggio può comportare una migliore gestione del dolorecite-ref-24[24]. Tuttavia nell'iperalgesia, così come in numerose altre sindromi nervose associate a dolore, la scelta del farmaco corretto o della giusta associazione tra più farmaci può risultare difficoltosa e richiedere una certa quantità di tentativi.
